Parish Membership Application Form

For new families and communicant members residing in Hebbal, Yelahanka, Sahakaranagar, Kodigehalli, and North Bengaluru seeking formal enrollment into Jerusalem Mar Thoma Church.

Requirements & Checklist:

Transfer Certificate (Kaikkaran / NOC) from previous parish
Communicant status and baptism details of all family members
Proof of current residential address within church jurisdiction
Signature of Head of Family & endorsement by Ward Prayer Leader

SACRAMENTS Aug 2026

Holy Baptism Application Form

Application for the Holy Sacrament of Christian Baptism for infants and believers, celebrated during regular parish worship in accordance with the rites of the Mar Thoma Syrian Church.

Requirements & Checklist:

Submission to Vicar at least 2 weeks prior to desired baptism date
Child's official birth certificate copy
Both parents must be registered communicant church members
Details of chosen Godparents (Sponsors) in good spiritual standing

SACRAMENTS Aug 2026

Notice of Holy Matrimony & Banns Request Form

Official notification of intention of marriage and application for publication of marriage banns in church prior to the solemnization of Holy Matrimony.

Requirements & Checklist:

Formal notice submitted to Vicar minimum 30 days in advance
Pre-marital counselling completion certificate
No Objection Certificate (NOC) / Free-state certificate from home parishes
Publication of Banns on 3 consecutive Sunday worship services

ADMINISTRATION Aug 2026

Inter-Parish Transfer Certificate (NOC) Request Form

Formal application for issuance of Transfer Certificate upon permanent or long-term relocation of a member or family from JMTC Hebbal to another Mar Thoma Parish.

Requirements & Checklist:

Verification of parish roll number and membership records
Clearance of monthly parish subscriptions and annual dues
Full destination parish name, diocese, and Vicar contact

STEWARDSHIP Aug 2026

Annual Subscription & Tithe Pledge Form

Commitment of monthly family subscription, harvest festival pledge, and voluntary missionary support towards church upkeep, pastoral welfare, and charitable outreach.

Requirements & Checklist:

Monthly family subscription commitment
Harvest Festival / Snehavirunnu annual pledge
Church development, charity, and cemetery fund contribution
Preferred contribution mode (UPI, NEFT/IMPS, Cheque)

ORGANIZATIONS Aug 2026

Organization / Sakha Membership Registration Form

Enrollment form to actively register and participate in parish organizations: Yuvajana Sakhyam, Sevika Sangham, Edavaka Mission, Senior Citizens Fellowship, Sunday School, or Choir.

Requirements & Checklist:

Selection of target organization(s)
Contact and WhatsApp communications opt-in
